1. Telephone Bill 2. Electricity Bill 3. Water Bill 4. Gas Connection
5. Bank Statements signed by the Bank
6. Service Tax /VAT/Sales Tax registration certificate
7. Property Tax/Corporation/Municipal corporation receipt
8. Driving License
9. Voter ID Card
10. Passport
11. Certificate of Registration for owned Vehicle

5. Authorization letter in favour of the certificate applicant from the Organisation as per format below
6. List of Partners/Members/Directors with their complete name and address details
